The contact form
When you send a message through our contact form, the name, phone number, email address and message you enter are stored in our website’s form system, provided by Wix, and sent to our office by email so that we can reply. The form is not a secure patient portal, so please do not include clinical details, diagnoses, or insurance or card numbers.

Our website is hosted on Wix. Wix, and analytics tools we may use such as Google Analytics, use cookies and similar technologies to keep the site working, count visits and understand which pages are useful.
